• Must be 16 years old with parent permission or 17 and older
• Must weigh at least 110 pounds
• Must know your social security number
• Must have eaten a healthy breakfast

You are unable to donate if:

• You have a tattoo that is less than a year old
• Have a cold
• If you do not have a 16 year old parent permission slip form signed by your parent
