Informational meetings will be held at the junior high schools and at Temple Hill Academy and Meadow Hill Global Explorations Magnet School for 7th graders. Announcements about tryouts and practice will be made and posted in the schools.
All prospective student athletes must have a Newburgh Health Appraisal Form (physical) completed by their family physician or by the school nurse practitioner or school physician. Forms can be obtained in any school health office. Return the completed Health Appraisal Form to your child’s health office at school to obtain a yellow clearance slip. No student will be permitted to tryout or participate unless they have obtained a yellow clearance slip from their school health office. The yellow clearance slip will then be presented to the coach on the first day of tryouts.
To obtain your
yellow clearance slip, you will be asked to leave your forms and you will be given a date and time to return for your clearance slip. This will give the health office staff sufficient time to obtain and review the student athlete’s health records.

What is Selection/Classification?
The Selection/ Classification Program is a process for screening students to determine readiness to compete in interscholastic athletic competition by evaluating their physical maturity, fitness, and skill. The intent of this program is to provide for students in grades 7 through 8 to safely participate at an appropriate level of competition based upon readiness rather than age and grade. This program has been designed to assess a student’s physical maturation, physical fitness and skill, so the student may be placed at a level of competition which should result in increased opportunity, a fairer competitive environment, reduced injury and greater personal satisfaction. The Selection/ Classification Program was designed for mature and exceptionally skilled students to advance to an upper level. The program is not to be used to fill positions on teams, provide additional experience, provide a place for junior high students when no modified program is offered, or to reward a student. It is aimed at the few select students who can benefit from such placement because of their level of readiness.
Parents-
• Sign Attachment “C” Parental Permission Form.
• Sign the Newburgh Health Appraisal Form.
• Sign the Parental Authorization Form.
• Pick up Attachment “E” (female) or Attachment “D” (male) entitled Developmental Screening from the Athletic Office.
• Return all completed forms to your child’s Health Office. (NFA Health Office in the summer)
• Our school physician must approve the physical exam and the results of the Developmental Screening.
• The Health Office will issue your child a “yellow clearance slip” if all is in order.
• If your child meets the physical maturity requirements for his/her particular sport, then a Physical Fitness Test will be administered on a date noted on the enclosed sheet.
• Passing the physical maturity test and the physical fitness test qualifies your child to tryout for the team.
