Weekly Student Health Questionnaire
One of the health and safety requirements for students who are currently receiving in-person instruction is to complete a health questionnaire every week.
Scholars in the Gold group should complete the form on Wednesdays.
Scholars in the Gold group should complete the form on Wednesdays.
Scholars in the Bold group should complete the form on Sundays.
The student health questionnaire should be accessed using the student's email address (ending in @students.necsd.net) and is located on students’ Launchpad or can be accessed through the District’s website under 'Important Topics' as well as under the 'Families tab.' For our younger scholars, we suggest families assist in the completion of the questionnaire. Additionally, during the days students are in school, temperature checks will take place daily.

Uno de los requisitos de salud y seguridad para los estudiantes que actualmente reciben instrucción en persona es completar un cuestionario de salud cada semana.
Los estudiantes del grupo oro (Gold Group) deben completar el formulario los miércoles.
Los estudiantes del grupo azul (Blue Group) deben completar el formulario los domingo.
El cuestionario de salud se ubicará en el Launchpad de los estudiantes o se puede acceder a él a través del sitio web del Distrito en Temas importantes, así como en la pestaña Familias. Los estudiantes deben iniciar sesión en su cuenta de correo electrónico de estudiante (@ Students.necsd.net) para completar el formulario. Para nuestros estudiantes más jóvenes, sugerimos que las familias ayuden a completar el cuestionario. Además, durante los días que los estudiantes están en la escuela, se realizarán controles de temperatura a diario. ¡Gracias por hacer su parte para mantener a todos seguros y saludables!
