Schedule NECSD Student Vaccination (Pfizer), 12+ - September 20th
The Newburgh Enlarged City School District is planning to host vaccine distributions in the coming weeks. If interested, please use a link below to schedule an appointment.

Scholars who are interested can share this form with family members if they would like to receive the vaccine together.
If you are able to complete the consent forms prior to your appointment time, that will expedite the check-in process. Consent forms can be found here.
The clinics have been made possible by several school nursing and teacher volunteers, as well as current and alumni members of the Certified Nursing Assistant Program. We are so thankful for their generosity and expertise! These distributions are in collaboration with Middletown Medical.

Tanto el estudiante vacunado como el padre / tutor que lo acompaña deben traer una identificación adecuada (certificado de nacimiento, tarjeta de seguro social, identificación del padre, pasaporte, identificación del consulado, licencia de conducir, identificación municipal, etc.) para recibir la vacuna. Traiga la información del seguro si la tiene. Si no tiene seguro, está bien, aún podrá vacunarse.
Los académicos que estén interesados pueden compartir este formulario con miembros de la familia si desean recibir la vacuna juntos.
