Synchronous Academy

We've designed our Synchronous Academy to provide as seamless an educational experience as possible for our scholars. Our goal is to have each synchronous student assigned to a traditional classroom. They will then receive instruction through their teachers google classroom synchronously as their in-person classmates are being taught by the same teacher. We believe this will provide the best learning experience for our scholars.

The application must be submitted by Friday, August 27, 2021  

 

 

 

 

How To Apply

1. Download Eligibility Form

Students who meet the medical fragility requirements, or have a household member, may apply for synchronous learning by using the Eligibility Form below. Part 1 of the form should be completed by the students parent or guardian.

2. Contact Medical Provider

Contact a licensed medical provider to complete Part 2 of the COVID-19 Health Eligibility Form. Licensed medical providers include Medical Doctor (MD), Doctor of Osteopathic Medicine (DO), Physician Assistant (PA) and Advanced Practice Registered Nurse (APRN). 

 

  

3. Submit Eligibility Form

Please complete the form in its entirety and either email to Sharon Patsalos, Health Services Facilitator at spatsalos@necsd.net OR drop off at the Board of Education Building (124 Grand Street, Newburgh, NY) to the attention of Sharon Patsalos. You can also call 563-3497. The application cannot be processed until all required documentation is submitted.

 

 

 

 

 

 

 

Medically Fragile Conditions

Enrollment will be limited to students, or students with a household member, with health or medical needs documented by a healthcare provider. See below to find out if your student falls under this criteria. 

Below are some questions to ask yourself to determine if your child may qualify as Medically Fragile:

Is the condition characterized as a chronic and complex physical condition?

Does the condition require prolonged dependency on specialized medical care?

Does the condition require frequent daily skilled nursing interventions?

Would interruptions or denial of daily skilled nursing services possibly lead to irreversible damage or death?

Has the condition required hospitalizations over the past 12 months ?

For classified students or students with 504 plans, does your child have documentation of impact on their health and safety and access to learning?

If you could not answer YES to any of the above questions, your child probably does not qualify as medically fragile. 

Examples of conditions that qualify as medically fragile:

  • Uncontrolled Seizures
  • Chronic and Severe Respiratory Abnormalities 
  • Congenital Heart Disease
  • Microcephaly or hydrocephalus
  • Immunocompromised Conditions
  • Muscular Dystrophy or Muscular Atrophy

Examples of conditions that DO NOT not qualify as medically fragile:

  • Mild Asthma
  • Allergies
  • Unvaccinated Status
